If you receive the "failed to get user - required number of reset answers not provided" error, follow these steps:
- Confirm that each field in the Password Reset Tool is being filled out correctly (username, new password, retype new password, legal first name, birthday, and last 4 digits of SSN).
- Close current screen and try again in a new browser (Internet Explorer, Firefox, or Safari).
