FSU seeks to support the diverse identities and experiences of our community members. We seek to collect current demographic and identity information that assists us in learning more about those represented on our campus. Our goal is to develop supports, resources, and practices that honor the diversity of our community.
With the Biographic-Demographic Change request form, you can update/correct your:
- Address
- Date of Birth
- Legal Name
- Preferred First Name and Pronouns
- SSN/TIN
- Employment Information
- Emergency Contact
- Religious Affiliation
- Sexual Orientation or Gender Identity
- Legal Sex
- Ethnicity and Race
Please know that with many of these update/corrections, legal documentation will be required and can be upload in the form.
